Can a Healthy Lifestyle Prevent IBD?

Adopting and maintaining a healthy lifestyle may prevent inflammatory bowel disease (IBD), according to findings from an analysis of three prospective U.S. cohort studies, which were validated in three external European cohorts.

In the primary analysis, maintaining low modifiable risk scores — based on risk factors including body mass index, smoking status, use of non-steroidal anti-inflammatory drugs, physical activity, and daily consumption of fruit, vegetables, fiber, polyunsaturated fatty acids, and red meat — could have prevented 42.9{bf0515afdcaddba073662ceb89fbb62b6b1bf123143c0e06b788e1946e8c353f} of Crohn’s disease cases and 44.4{bf0515afdcaddba073662ceb89fbb62b6b1bf123143c0e06b788e1946e8c353f} of ulcerative colitis cases, reported Hamed Khalili, MD, MPH, of Massachusetts General Hospital in Boston, and colleagues.

Moreover, adherence to a healthy lifestyle could have prevented 61.1{bf0515afdcaddba073662ceb89fbb62b6b1bf123143c0e06b788e1946e8c353f} of Crohn’s disease cases and 42.2{bf0515afdcaddba073662ceb89fbb62b6b1bf123143c0e06b788e1946e8c353f} of ulcerative colitis cases, they noted in Gut.

“We were surprised by the proportion of cases that could have been prevented through lifestyle modifications,” Khalili told MedPage Today. “One reason for this finding may be that our population was older and therefore most of our cases of IBD happened in older adults.”

“We know that lifestyle and environmental factors play a bigger role in the development of IBD in this population as compared to those who are diagnosed with disease earlier in life,” he added.

These findings from the Nurses’ Health Study (NHS), the NHS II, and the Health Professionals Follow-Up Study (HPFS) were largely validated in three external European cohorts — the Swedish Mammography Cohort, Cohort of Swedish Men, and the European Prospective Investigation into Cancer and Nutrition.

Looking at cases of Crohn’s disease among the European validation cohorts, adhering to low-risk factors could have prevented 44{bf0515afdcaddba073662ceb89fbb62b6b1bf123143c0e06b788e1946e8c353f} to 51{bf0515afdcaddba073662ceb89fbb62b6b1bf123143c0e06b788e1946e8c353f} of cases, while adhering to a healthy lifestyle could have prevented 49{bf0515afdcaddba073662ceb89fbb62b6b1bf123143c0e06b788e1946e8c353f} to 60{bf0515afdcaddba073662ceb89fbb62b6b1bf123143c0e06b788e1946e8c353f} cases. For ulcerative colitis, adherence to low-risk factors could have prevented 21{bf0515afdcaddba073662ceb89fbb62b6b1bf123143c0e06b788e1946e8c353f} to 28{bf0515afdcaddba073662ceb89fbb62b6b1bf123143c0e06b788e1946e8c353f} of cases, while healthy lifestyle adherence could have prevented 47{bf0515afdcaddba073662ceb89fbb62b6b1bf123143c0e06b788e1946e8c353f} to 56{bf0515afdcaddba073662ceb89fbb62b6b1bf123143c0e06b788e1946e8c353f} of cases.

For every 1-point increase in modifiable risk score, a higher risk of Crohn’s disease (P for trend<0.0001) and ulcerative colitis (P for trend=0.008) was observed, and was similar for men and women.

IBD affects about 3.1 million people in the U.S. and 1.3 million in Europe, with incidence rising globally, especially among newly industrialized countries, Khalili’s group said. IBD is associated with an annual healthcare cost of $23,000 per patient in the U.S., and there are no current strategies to prevent the development of IBD. While one approach to preventing many chronic diseases is modification of lifestyle and dietary factors, the success of adhering to such changes remains unclear.

For this study, Khalili and colleagues examined data on 72,290 participants from the NHS, 93,909 from the NHS II, and 41,871 from the HPFS. The NHS enrolled female nurses ages 30 to 55 across 11 states in 1976. NHS II assessed a slightly younger cohort of female nurses (ages 25 to 42) from 15 states starting in 1989, while the HPFS enrolled male physicians ages 40 to 75 across all states in 1986.

In order to externally validate their findings, the researchers assessed data on 40,810 participants in the Cohort of Swedish Men, 404,144 from the European Prospective Investigation into Cancer and Nutrition, and 37,275 from the Swedish Mammography Cohort.

Using participant baseline and biennial questionnaires that assessed lifestyle factors, anthropomorphic data, and medical history, Khalili and colleagues developed modifiable risk scores ranging from 0 to 6 for Crohn’s disease and ulcerative colitis, with higher scores indicating more risk factors. Healthy lifestyle scores ranging from 0 to 9 were also developed, based on recommendations from the American Heart Association and other organizations, with higher scores indicating a healthier lifestyle.

A healthy lifestyle included never smoking, a BMI between 18.5 and 25, and engaging in physical activity of at least 7.5 metabolic equivalent of task-hours per week, in addition to consuming less than half a serving of red meat per day, at least eight daily servings of fruit/vegetables, at least half a serving of nuts or seeds per day, at least two servings of fish per week, at least 25 g of daily fiber, and a maximum of one daily alcoholic beverage for women and two for men.

Across 5,117,021 person-years of follow-up, 346 cases of Crohn’s disease and 456 cases of ulcerative colitis were reported.

On falsification analysis, adherence to low-risk factors for Crohn’s disease could have also prevented 32.3{bf0515afdcaddba073662ceb89fbb62b6b1bf123143c0e06b788e1946e8c353f} of cases of rheumatoid arthritis, 13.3{bf0515afdcaddba073662ceb89fbb62b6b1bf123143c0e06b788e1946e8c353f} of cases of colorectal cancer, and 14{bf0515afdcaddba073662ceb89fbb62b6b1bf123143c0e06b788e1946e8c353f} of cases of cardiovascular disease, though this was not the case for ulcerative colitis.

“This is largely due to differences in strength of associations and prevalence of risk factors, and presence of other modifiable risk factors such as alcohol and medications or supplements which are strongly associated with these other conditions,” Khalili and colleagues suggested.

They acknowledged that younger-onset IBD was under-represented in their study, since the mean age of the cohort (about 45) was higher than the usual age at onset of IBD. In addition, data on other modifiable risk factors such as stress were not explored, and high-risk individuals were not assessed.

At the conclude of the night, I have a lot more time for myself, and I conclude up utilizing Will Accomplish item simply because it has magnesium in it, which assists reduce, which tends to make me come to feel additional peaceful, enabling me to get a great night’s slumber, but I have to have to go and commit a day at the spa. So, is Stutz right?

Leveraging consumer brands to bring healthy lifestyle options to seniors

At one time, the business of healthcare might have seemed impenetrable to outsiders. If an organization wasn’t directly connected to medicine, research, pharmaceuticals, health insurance and the like, there was no place for it. However, as stakeholders have recognized the assets those outside of the field can contribute, there has been a surge of collaborations and integrations that strengthen healthcare delivery, quality and outcomes.

For example, while physicians have long touted the benefits of physical activity, health plans also understand the value of including gym memberships or fitness programs as a benefit. Don’t think that just because you live a healthy lifestyle you are immune to lung cancer.

Don’t think that just because you live a healthy lifestyle you are immune to lung cancer.

(Trent Nelson | The Salt Lake Tribune) Poor air quality obscures the Oquirrh Mountains and downtown Salt Lake City, as seen from the University of Utah on Friday, Dec. 3, 2021.

We are two women with one too many things in common. We both lived our adult lives in Salt Lake City, are nurses, college professors, were never exposed to radon or occupational hazards and are non-smokers. We shared the mistaken belief that our healthy lifestyles would protect us from serious diseases.

The one-too-many things we have in common is that we were both discovered to have advanced non-small cell lung cancer caused by a common genetic mutation activated by Salt Lake valley air pollution.

Even though we were/are college of nursing professors and read medical literature, neither one of us realized breathing polluted air put us at higher risk for lung cancer, according to the CDC. We blithely assumed non-smokers didn’t get lung cancer. Too late we learned many studies have shown long-term air pollution exposure is toxic enough to cause lung cancer in people like us.

We learned the hard way that, in most cases, lung cancer can remain early stage and asymptomatic for many years and solid tumors usually take decades to develop. But in 2013 lung cancer in an 8-year-old girl in China, attributable to air pollution, set off alarm bells throughout the world.

In 2019, 300,000 lung cancer deaths globally were linked to exposure to outdoor air pollution. According to the CDC, up to 20{bf0515afdcaddba073662ceb89fbb62b6b1bf123143c0e06b788e1946e8c353f} of lung cancers occur in non-smokers.

We learned our ignorance isn’t uncommon. We were both initially worked up by our family physicians for other possible reasons for our fatigue, persistent cough and shortness of breath with exercise, delaying our diagnoses and treatment. This is typical of lung cancer, the number one cancer killer in the U.S.

Because non-smokers aren’t screened for lung cancer, and the early-stage symptoms are subtle and self-reported, it isn’t usually diagnosed until Stage III or IV, when it is much more difficult to treat, and survival is low — exactly our situation. Too late we also learned that air pollution also increases the size and grade of lung cancers, and even impacts our ability to survive by decreasing the effectiveness of our treatment and by making us more susceptible to recurrence.

Fortunately for us, there is an oral chemotherapy medication we take that can extend our lives by a few years, but this isn’t the way we expected to live them, with debilitating symptoms, extensive medicine-generated side effects and very expensive medical costs.

The World Health Organization has declared air pollution a leading cause of cancer, and there is no safe level of air pollution, just like there is no safe number of cigarettes a person can smoke. But air pollution also causes a list of other diseases nearly as long as those from smoking — virtually every type of lung disease, heart and blood vessel diseases, impaired fetal development, chromosomal damage, pregnancy complications, brain and neurologic disorders like strokes and Alzheimer’s, type II diabetes, accelerated aging and premature death.

We share our stories in the hope you and your health care providers won’t wait until it is too late to learn more about the negative consequences of chronic exposure to air pollution. Lung cancer is a real possibility, and it shouldn’t be considered only after everything else has been ruled out. It is only one of many poor health outcomes that needs to be considered when symptoms first appear in non-smoking patients.

We also issue a call to arms for all who live in this beautiful state to work with our elected officials to change public policy and to fight against proposals that will increase air pollution, such as the inland port and water rights causing Great Salt Lake to become a dry, toxic dust generating wasteland. We must advocate for healthier air to protect ourselves and those who come after us, and an end to public policy that benefits the few at the expense of the many.

Behind every abstract statistic on the health hazards of air pollution is a human face — someone’s spouse, mother, father, grandparent, brother, sister or newborn baby — someone who was denied a long, healthy life simply because of the air they breathed. We hope hearing our stories and seeing our faces can lead to fewer stories like ours.

Holiday eating stress you out? Here are some helpful tips!

By Chrissy Krueger RN, RD, CDN, CDCES, Diabetes Education & Care Specialist, Oak Orchard Health

As a Diabetes Educator, at Oak Orchard Health, I hear a lot about the anxiety and stress of overeating during the holidays. Whether it’s Thanksgiving, Christmas, or another seasonal holiday, learning to eat healthy during this time can be a challenge – but you can do it. Keep in mind that each holiday represents just one day, and you can get back on your healthy meal plan the next day.

Here are a few tips to help you get through the holidays feeling good about yourself.

Start by preparing in advance.

If the party is not at your home, consider asking the hostess if you can help by bringing a dish to pass. That dish will be a healthy one for you and delicious for others. That’s your “go-to” dish when you’re feeling hungry or other options are not too appealing.

Skipping meals before the holiday dinner isn’t a good idea.

Skipping meals can result in moodiness or irritability. For some, it can also lead to low blood sugar. You should not get yourself so hungry that you will eat anything and everything!  Try to eat regularly throughout the day, maybe smaller meals than usual, but do eat breakfast and lunch.

Re-think the holidays.

Why are you gathering? To be with family and friends? Sharing experiences with each other? The point is that holidays aren’t just about food. In fact, after eating your meal, avoid lingering near the holiday spread. Go into another room to mingle.  Also, consider taking a walk with someone you haven’t seen in a while.

Avoid taking leftovers home.

Unless a loved one was unable to attend and you want to bring them some treats, avoid bringing leftovers home. If you’re the hostess, don’t make too much food and if you do, have a plan for giving it away to your guests or a local senior center or food shelter.

Don’t give up your healthy lifestyle.

Often people use Thanksgiving as a reason to start overeating throughout the holiday season. Try to think of each holiday as a one-day event and get back on track with your healthy habits the next day. You’ll feel better!

Heart-healthy portions.

Whether you have diabetes or not, these are great guidelines on how to fill your plate. A good rule of thumb is that half your plate should contain non-starchy vegetables such as green beans, broccoli, or cauliflower, a quarter of the plate with protein, and a quarter with carbohydrates. Riced cauliflower is very popular and can be substituted for mashed potatoes – or make half the mashed potatoes with real potatoes and the other half with riced cauliflower. Your guests won’t know the difference!

Desserts.

If you have room for dessert, try to limit your portion sizes. An eighth of a pie or 2” square of cake can be enough. If you’re going to bring a dessert, choose one that has fewer carbohydrates and sugar. People with diabetes can eat desserts but it’s all about the size of the portion. The smaller the better.

Always carry a glass of water.

Having a glass of water and sipping it throughout the day will help you avoid picking on food. This will also help to reduce your alcohol consumption.

No thank you.

Be comfortable saying no thank you when offered another portion.  Those who truly love you will not change their mind based on how much or what you eat!

Holidays are not just for eating.

If you’re the hostess, you can plan to do something with your guests like playing games or taking a walk. Plan to have everyone step away from the table and go to another room to play a game like charades or a card game. The more active the better.

If you do overindulge, don’t beat yourself up about it. Just get back on track. Take a walk the next day and start eating healthy again.  Remember, don’t skip meals either!
